Instructions:
1. Complete the form below in its entirety and submit to Ferguson Hall Room 100 (even if you have previously takensecondary guitar lessons here).
2. You will not be permitted to register until you have been assigned to an instructor. Once the assignments are made, you will be entered for permission in the appropriate section. You will then receive an e-mail with your instructor’s name and all of your registration information. Please contact the School of Music Student Services Office at (612) 624-5740 with any questions/concerns regarding registration.
3. We will make every effort to honor requests for a specific instructor.
4. The applied music fee is $150 for 15 half-hour lessons with a graduate TA; this fee will automatically be charged to your student account when you register for lessons.
